I hope that project answers the problem I had of building the Base64 encoded data structure sent in the header. I was using a cheap Java class that I wrote to do it. 

Turns out this was all scrapped due to some weird circumstance with the IBM Apache web server (IHS). Still never figured it out and in fact we have an ongoing open ticket with IBM looking into it. I believe our servers are setup incorrectly. My solution worked everywhere except through IE6 - which is a damned requirement. 

Anyways we are still using LDAP but not NTLM. Which is kind of a shame because I liked tackling that beast.

I'm glad it helped out and I totally agree this should be a plugin.</description>

It looks like the RubyNTLM  gem on RubyForge implements some of your Pseudocode above. Here's a sample from their readme:
